The Senior Services Collaborative (SSC)
The Senior Services Collaborative (SSC), facilitated by Aging Together, was designed to help community organizations focus and align their efforts to optimize senior services and supports in the area that represents the PATH Foundation footprint – Culpeper, Fauquier and Rappahannock Counties. Through quarterly meetings and workgroups community agencies hear from local experts about housing, transportation, access to food, social isolation and other issues facing older adults and then respond with innovative solutions.

In 2020 – 20I21, with funding from the PATH Foundation, the following initiatives were launched in partnership with the Senior Services Collaborative:
- Homecare Companion Program – This project is a hands-on pilot designed to test, adapt and refine the framework for providing in-home care facilitated by Rapp at Home, a rural neighbor-to-neighbor program. The pilot was developed in coordination with DSS, People Inc., Aging Together and PATH.
- Meal delivery program – Fauquier FISH, the host agency in partnership with Fauquier County Department of Social Services (DSS), received PATH funding for a meal delivery program for isolated seniors modeled after FISH programs for children.
- Creation of the Regional Resource Guide for Older Adults - Aging Together in partnership with PATH, Rappahannock Rapidan Community Services (RRCS) and other partners created a Resource Guide tailored to older adults and their caregivers. The first of its kind in the region, the Guide includes a Spanish language section.
- Generations Central Adult Day - Located at Culpeper Baptist Church, the steering committee has been meeting for two years and represents a collaborative effort of Culpeper Baptist Church, Rappahannock Rapidan Community Services, George Mason University, Aging Together, and PATH.
- Project Lifesaver for the Warrenton Sheriff’s Department

- A behavioral Health Screening tool, through RRCS, for older adults
- Creative Interventions for Caregiver Training with George Mason University
