This page contains partnership documents including a strategic plan and goals for the organization. The Aging Together partnership is as diverse as the five counties it serves. The partnership includes businesses, organizations, agencies, civic clubs, individuals, and state and local government. The initial stages of planning began with a two year grant from the Robert Wood Johnson Foundation (RWJF). With that grant came the development of a needs assessment, strategic plan, goals, and a solid foundation for the partnership.  Once completed, the Core Leadership applied for and was awarded a four-year implementation grant also through RWJF. Aging Together was one of eight organizations nationally selected to be awarded this four-year implementation grant. Aging Together continues to strengthen its diverse membership while achieving its vision.                   

Aging Together envisions a two-fold impact on our communities:

• Older adults and their families will have access to an expanded range of options for long term care and supportive services (with Aging Together serving as a catalyst and resource developer)
• Communities and decision-makers will value older adults and their families and consider their needs and preferences in all local planning (with Aging Together serving as a resource and source of information to assist decision-makers)
   

The partnership’s objectives addresses primary areas of concern to seniors and their families including:

• Establish an effective and sustainable volunteer network
• Increase the capacity of the long-term care workforce
• Facilitate the increase of long-term care and support services in the region
• Increase knowledge among older adults and caregivers about long term care options
• Increase community awareness and response to needs of older adults
• Assure vitality and sustainability of county and regional partnerships
   

With its broad participation and emphasis on collaboration and coordination, Aging Together is changing the way we do business in the region:

• Pooling resource to do things no one group could do on its own
• Speaking with one voice on aging issues
• Working together to generate new resources and expand services
• Providing expertise to help communities plan for the future
